Communication between school and home is essential for the success of a special needs student. This daily-behavior-chart.xls is an example of an Excel worksheet that teachers can use to evaluate a student’s behavior during the school day. The document can be e-mailed to parents or printed and sent home with the student each day. Although this chart is geared toward a student’s physical aggression toward others and his or her focus on tasks, it can be modified to track any behavior of interest. Here is another example that allows students to self evaluate in addition to the teachers’ evaluation (behaviorchart.xls). Another format for communication ia a weekly-feedback-form.doc that can be sent back and forth between school and home throughout a given week. This example tracks a students impulsiveness and overreaction during the school day, but the behaviors of focus can be changed to fit the needs of any student.
